Malex Tablet: Each contains Sulphadoxine BP 500 mg & Pyrimethamine BP 25 mg.

Malex (Sulphadoxine & Pyrimethamine) is an antimalarial agent which acts by reciprocal potentiating of its two components, achieved by a sequential blockade of two enzymes involved in the biosynthesis of folinic acid within the parasites. The components of Malex are both orally absorbed and excreted mainly by the kidneys.

Malex is indicated in the treatment of acute malarial attack particularly caused by Plasmodium falciparum. Also, Malex is indicated for prophylactic management to the travellers in the area where resistant to Chloroquine is endemic. Unlabeled uses: Malex can be used as a prophylactic agent for the prevention of Pneumocystis carinii pneumonia in patients with AIDS usually as a second line agent.

Recommended Dosage to be administered as: Adult: over 14 years of age: 2-3 tablets as a single dose; Child under 4 years: ½ tablet; age 4-8 years: 1 tablet; 9-14 years: 2 tablets; All as single dose. Prophylaxis: over 14 years: 1 tablet weekly, Child under 4 years: ¼ of adult dose; 4-8 years: ½ of adult dose; 9-14 years: ¾ of adult dose.

Malex is contraindicated in patients hypersensitive to Sulphadoxine or Pyrimethamine with documented megaloblastic anaemia due to folate deficiency, infants under 2-months of age, pregnancy at term and during the nursing period, prophylactic use in patients with severe renal insufficiency, marked liver parenchymal damage or blood dyscrasias.

Periodic blood counts should be performed and urine should be analyzed for crystalluria during prolonged prophylaxis. Also, the drug should be administered cautiously to patients with possible folate deficiency, severe allergy or bronchial asthma.

Side effects are hypersensitivity reaction, some haematologic symptoms, GIT symptoms and CNS Symptoms.

Sulphadoxine & Pyrimethamine is contraindicated during pregnancy, premature and newborn infants during the first weeks of life and intolerance to sulfonamide. If pregnancy cannot be excluded, possible risks should be balanced against therapeutic effect. Both Pyrimethamine and Sulphadoxine are excreted in maternal breast milk. Nursing mother should not take this preparation.
